The ideal candidate will have a profound understanding of pharmaceutical therapy, a keen eye for detail, and the ability to work effectively in a team-oriented setting.
Responsibilities
- Evaluate and interpret physician orders for appropriateness and accuracy, ensuring all prescriptions are dispensed correctly.
- Provide patient counseling and medication therapy management services.
- Monitor patient drug therapies, including over-the-counter (OTC) medications and dietary supplements, for drug interactions, contraindications, and adverse reactions.
- Develop, implement, and assess pharmaceutical care plans for patients.
- Collaborate with healthcare team members to optimize patient care, participating in rounds and making recommendations for drug therapy changes as necessary.
- Participate in the development and delivery of educational programs for the healthcare team, patients, and their families.
- Maintain up-to-date knowledge of new drugs, medical device regulations, therapies, and treatment protocols.
- Ensure compliance with all federal, state, and local laws and regulations related to the dispensing of drugs and controlled substances, and the maintenance of required records.
- Use the Epic system for maintaining electronic health records and managing pharmacy operations.
- Doctor of Pharmacy degree (PharmD) from an accredited institution.
- Current, unrestricted Pharmacist license in good standing.
- Minimum of 2 years of experience as a Clinical Pharmacist in a hospital or similar medical setting.
- Proficiency in the use of Epic system for managing pharmacy operations.
- Experience with 457b plans is preferred.
- Excellent verbal and written communication skills.
- Strong analytical skills and ability to make decisions based on clinical judgment.
- Ability to work effectively in a fast-paced, high-stress environment.
- Demonstrated commitment to providing outstanding patient care.
- Knowledge of current pharmaceutical practices and clinical pharmacy standards.
- Proficiency in Microsoft Office Suite, particularly Excel and Word.
- Ability to work flexible hours, including nights, weekends, and holidays as needed.
