**This position is on-site at our facility located in Bloomfield, Indiana
SEE Purpose Addictions Treatment is a premier drug rehab facility in Indiana, offering multiple levels of care including sub-acute detox, residential inpatient treatment, and after-care planning.
The case manager works with dually diagnosed adults who have both substance use disorder and co-occurring mental health disorders. This position provides ongoing case management services for adults 18 years of age and older who are experiencing severe and persistent substance use problems and mental illness. The case manager works with the Clinical Director and the client’s primary therapist to provide comprehensive services to clients while in treatment. This position also provides group facilitation, and observes and interacts with clients on a regular basis to provide support and address issues related to their treatment experience and aftercare needs.
Essential Duties and Responsibilities
Essential duties and responsibilities of the case manager include the following:
1. Provides on-going support for clients who have a severe and persistent substance use disorder and mental health problems.
2. Provides case management services and completes necessary communication and updates as required to Clinical Director, Case Management supervisor, COO and CMO.
3. Provides effective and efficient management of caseload.
4. Participates in peer consultations, staff meetings, and continuing education. Case management notes and updates need supervisor’s signature. Charts need to be presented to supervisor on a regular basis.
5. Facilitates referrals to appropriate internal and/or external resources where indicated.
6. Provides consultation and education service to individual and community groups as requested and possible.
7. Participates in program development as requested and where possible.
8. May assist in training and supervision of students or staff.
9. Keeps current with trends and developments related to essential job competencies, industry development, and innovation.
Supervisory Responsibilities
This position has no direct supervisory responsibilities.
Education and/or Experience
Associate degree in addiction, counseling, social work or a related behavioral/mental health field from an accredited college or university is desired.
Certificates, Licenses, Registrations
Must maintain a current and valid certification as an ACIT, CADAC or other equivalent certification in the State of Indiana.

While performing the duties of this job, the employee is regularly required to stand; walk; sit; use hands to finger, handle, or feel; stoop, kneel, crouch, or crawl and talk or hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 20 pounds. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception and ability to adjust focus.

This job operates in a residential healthcare setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses.
