Position Description: Home Care Aide

Status: Part-Time No Benefits: 23 or less hours per week. 

Shift: Day - 8:00 a.m. to 4:30 p.m.

Days: Monday thru Friday; including 1 weekend per month

Position SummaryProvides personal care and supportive services to home care clients in their place of residence to promote rehabilitation, safety, and independence within the individual's physical and mental limitations.
Education/Qualifications: High School Diploma or equivalent; Must have 75 Hour CNA training and certification. Requires current driver's license and auto insurance, with the ability to travel throughout Boone County.
Essential Functions:
  • Regular attendance.
  • Excellent verbal and written communication skills
  • Possess or have access to reliable transportation for all of Iowa's seasons
  • Ability to think and act independently when required
  • Confidence in providing personal cares and assisting with ADL's
  • Knowledge of computer and mobile devices for training and documentation purposes
  • Excellent organization and time management skills
  • Consistency in keeping on schedule and attending appointments on time according to home care's policies and procedures.
  • Demonstrates knowledge and skills necessary toprovide care appropriate to the ages of the patients served within thedepartment.
  • Maintains performance in compliance with Stateand Federal laws, and consistent with high standards of business and professionalethics.
  • Provides personal care assistance to patientssuch as bathing, grooming, dressing, simple wound cares, ambulating, andrehabilitation.
  • Assists patients with oral medications asdirected by home care professional.
  • Provides meal planning and preparation asdirected by supervisory staff.
  • Provides basic light housekeeping such as sweeping,mopping, vacuuming, dusting, cleaning bathroom/kitchen areas, changing linens,and laundry.
  • Provides miscellaneous duties such as grocery shopping,errands, money management/bill paying, and parenting skills.
  • Documents services provided according todocumentation standards of the department.
  • Reports changes in patient condition tosupervisor.
  • Maintains effective communication with alllevels of supervision and staff.
  • Maintains HCA scheduling planner, and attendsregular scheduling meetings.
  • Attends and participates in department staffmeetings/in-services and hospital wide mandatory in-services.
WORKING CONDITIONS:

Hours may vary depending on patient census. The employee is regularly exposed to outside weather conditions including driving. The employee is also exposed to varied home environments including pets and household pests, and cluttered conditions. The employee is occasionally exposed to wet and or humid conditions, moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electric shock. The noise level in this work environment is usually moderate, but can vary with each home situation.

Vision must be correctable to view computer screens and read printed information. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision and ability to adjust focus.

Hearing must be in the normal range for telephone contacts and other conversations.
