POSITION SUMMARY
:

The Bucks County Housing Link Coordinated Entry system is a systemic approach to streamlining access to housing and services utilizing a common entry point, standardized assessment tool, and coordinated referrals to programs and services that best match the needs of families and individuals seeking housing assistance. The Project Coordinator, Operations is responsible for oversight, monitoring, and continued development of the Housing Link Coordinated Entry system and its programs or services which can include call center helpline, diversion, emergency shelter, street outreach, and housing programs. The position will have a particular focus on ensuring high quality services at the individual level in the Housing Link Coordinated Entry system. The Project Coordinator, Operations will operate through the lens of social determinants of health (SDOH) with the understanding of housing as a key SDOH that is vital to the long-term well-being of people experiencing housing instability. The Project Coordinator will report to the Manager of Homeless Services and will have a dotted line relationship to the Managed Care Unit in the Department of Behavioral Health/Development Programs (as it relates to the Community Based Care Management (CBCM) plan, that funds this position).


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
:

  • Actively monitors all parts of the coordinated entry system for gaps and barriers in services related to SDOH. When gaps and barriers are found, the Project Coordinator will initiate case conferencing and system integration facilitation to resolve the barrier in a manner that leads to long-term housing stability
  • Identifies system gaps and barriers to promote policy and procedure improvements
  • Completes regular on-site and virtual meetings with coordinated entry providers including shelter, street outreach teams, and others to promote optimal coordination of services and to assist in resolution of any barriers identified
  • Serves as lead coordinator as needed over vital coordinated entry components such as street outreach. As lead coordinator, ensures efficiency of services among multiple providers and assists in development and implementation of policies and procedures to promote such efficiency. This component will be in collaboration with the BHDP PATH (Projects for Assistance in Transition from Homelessness) coordinator.
  • Provides internal and external leadership in the implementation and oversight of the coordinated entry system (Housing Link)
  • Identifies and resolve issues related to implementation throughout Housing Link
  • Updates and ensure compliance with Housing Link policies and procedures
  • Facilitates and participate in Housing Link related committees and meetings
  • Serves as main point of contact for various Housing Link initiatives that require coordination of services across multiple service providers across the spectrum of SDOH including intergovernmental coordination
  • Completes all required reporting and evaluation including assisting with ensuring data quality standards are met through various data systems including the Homeless Management Information System (HMIS) to ensure robust outcome reporting and optimal coordination between service entities
  • Creates and maintain a data collection system of interventions/services provided to HealthChoices Members, as it relates to the CBCM plan as well as those served through PATH program and any other data elements as needed.
  • Create and maintain data to be used in outcomes reporting and provide outcomes reports in collaboration with the Managed Care Unit, as it relates to the CBCM plan.
  • Provide face to face and or virtual interactions with CBCM plan participants, as needed.
  • Regular site visits to Housing Link providers in the field (such as street outreach, shelter, providers of various housing interventions) for purposes of system level coordination and consultation around CBCM plan participants needs to promote stable housing.
  • Performs similar or related duties as assigned/required.

QUALIFICATIONS REQUIRED:

  • Associates or Bachelor’s Degree in Human Services related field, Social Work, or other related field. If no higher education, equivalent experience in case management related roles will be considered.
  • Must be proficient in the following software programs: Microsoft Word, Excel, Adobe Pro, and Outlook.
  • Strong interpersonal and leadership skills with proven ability to develop strong working relationships across the community, with colleagues, providers and stakeholders. Prior experience in leading service delivery teams preferred.
  • Direct case management experience in the field of Human Services required. Direct experience working with clients experiencing housing instability preferred.
  • Knowledge of and experience in homelessness, housing issues, behavioral health, and Medicaid population preferred, particularly in working with individuals with a serious mental illness and/or co-occurring disorders.
  • Must possess excellent writing, speaking, negotiation and systems thinking skills with the innate ability to think analytically when problem solving.
  • Must possess, at minimum, an intermediate level of proficiency in computer literacy.
  • Must possess and maintain a valid driver’s license and have the use of a personal vehicle.
