The Adult Case Manager provides necessary support and coordination of services to extend the ability of both Severe & Persistently Mentally Ill (SPMI) and non-SPMI persons over the age of 18 to live in the community in the most independent manner possible.

Responsibilities Include:

• Provides direct Case Manager services with SPMI and non-SPMI clients, their families, and the community at large
• Coordinate and participate in development of individualized comprehensive client treatment plans.
• Assists clients with basic money management; monitors client’s medical and psychiatric status; assists clients with daily living skills.
• Assists clients in identifying and accessing community resources.
• Provide coordination and assistance in crisis intervention and stabilization.
• Assist client in gaining formal and informal community and personal networking.
• Advocates for emotionally disturbed adults in our six-county area.
• Ensures the timely completion of all documentation related to providing and billing for services rendered.
• Maintain relevant documentation and provide statistical reporting.

• Able to resolve crisis situations in the most clinically effective manner.
• Must possess a valid Ohio driver license with acceptable driving record.
