We are seeking a dedicated Registered Nurse to join a growing organization in New Caney, TX. The Hospice RN Case Manager is responsible for providing compassionate, comprehensive, and patient-centered care to terminally ill patients in their homes or residential care facilities. This role involves coordinating and implementing individualized care plans to ensure the highest quality of life for patients and their families, adhering to all regulatory and compliance standards.
**Responsibilities:**
The Hospice RN Case Manager is responsible for providing comprehensive patient care by conducting thorough assessments and developing individualized care plans to ensure timely delivery of hospice services. This role serves as the primary contact for patients and families, coordinating care with the interdisciplinary team and managing patient caseload efficiently. The RN administers medications and treatments, monitors patient conditions, and provides necessary palliative care measures. Effective communication and accurate documentation are crucial, as the RN maintains detailed records, communicates with patients, families, and healthcare providers, and participates in team meetings and quality improvement initiatives. Additionally, the RN educates patients and families on disease progression and care processes, offers emotional support and counseling, and facilitates discussions on advance directives and end-of-life decisions. Ensuring compliance with regulations and hospice policies, the RN also participates in quality improvement efforts and reviews care plans and outcomes to enhance service quality.
