Thomas Jefferson EMS Council is Hiring a Community Outreach Coordinator Near Charlottesville, VA

Job Overview:
TJEMS Community Outreach Coordinator is responsible for supporting the planning and coordinating functions of the EMS Council services focused on Mobile Integrated Health/Community Paramedicine options for regional jurisdictions/EMS Agencies: identifying existing community resources in each jurisdiction within the TJEMS catchment area to assist TJEMS and the EMS Agencies in each jurisdiction plan and/or enhance non-urgent outreach services and telehealth options. This position is focused on information gathering and resource collaboration duties, not direct patient care. This position is funded for 1 year (to August, 2025); position relies on future funding to continue.

In collaboration with TJEMS Executive Director

- Develop and maintain a standardized set of quality indicators for mobile integrated health/community paramedicine programs in the region;

- Identify and/or develop appropriate patient education on chronic diseases including diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Stroke, pneumonia, slips/trips/falls assessment, opioid/substance use disorder (SUD), nutrition and more;

- Develop and maintain external collaborative relationships with local service providers; serve as a community liaison in order to identified perceived needs, promote TJEMS and MIH/CP goals, values and potential services offered; learn about available resources available to clients within the community to create a cohesive collaborative approach to client care and participate in community-based meetings and committees to meet the physical, psychological and/or social needs of potential patients;

- Conducts post-hospital discharge follow-up visits, including reviewing medication, health literacy and knowledge of current condition(s);

- Develop telehealth protocols and resource pool of appropriate physicians and advanced practice providers.

In collaboration with TJEMS Training Director

- Assists in developing education for EMS providers to act as a patient advocate and healthcare navigator to assist in connecting community members with community resources;

- Liaises with community stakeholders to identify education and behavior modification strategies and social support needed by patients and families.

- Assist in the development of communication materials (Street Sheets) for EMS providers on available resources within their jurisdiction.

Skills:
- Knowledge of principles and practices of emergency medical services systems at the local and state levels.

- Understanding of planning principles and performance improvement concepts.

- Ability to establish and maintain effective working relationships with community organizations, hospital personnel, and EMS agencies.

- Excellent written and oral communication skills.

- Understanding of personal computers for business use and in the use of word processing, spreadsheet and database management software.

- Ability to adapt to a flexible, demanding schedule of working hours that involve travel throughout the region during day and evening hours with some weekend travel.

Education and Experience

Minimum – Two years of professional experience planning, developing, evaluating or operating a community-based program, and a bachelor’s degree in public health, public or business administration, emergency medicine, nursing or a related health field;

OR three years of professional experience planning, developing, evaluating or operating a community-based program, and an Associate’s degree in public health, human services, public or business administration, emergency medicine, nursing or a related health field;

OR four years of professional experience planning, developing, evaluating or operating a community-based program, and a certificate in public health, human services, public or business administration, or a related health field;

AND two years of experience in the direct delivery of emergency medical services as an EMT or higher.

Job Types: Full-time, Part-time, Temporary

Pay: $50,000.00 - $55,000.00 per year

Expected hours: No more than 40 per week
