Coordinates the admitting and transferring of residents to senior living AND the Adams Memorial Swing Bed program. Strives to provide an environment conducive to resident safety and comfort. Demonstrates competence in general nursing skills and is conversant in Medicare, Medicaid, and State/Federal guidelines for long-term care facilities. Maintaining a working relationship as a liaison with discharge planners, insurance carriers, physicians, and families. Maintains compliance with initial prior authorization for resident care. Must have a willingness to have flexible hours to meet the needs of the hospital and nursing facility. Performs other duties as assigned.

20 hours per week - First Shift
10:00 am - 2:00 pm
Weekend Commitment: As Needed
Current RN license in the State of Indiana or Social Worker (SW)
CPR certification is required
