Overview:
We are seeking a compassionate and skilled pediatric Home Care lpn to join our team. As a Home Care Nurse, you will provide personalized medical care and support to individuals in the comfort of their own homes. Responsibilities:
- Conduct comprehensive assessments of patient's health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed
- Monitor vital signs and observe for any changes in patients' conditions
- Provide wound care and assist with dressing changes
- Assist with activities of daily living, including bathing, grooming, and mobility
- Educate patients and their families on managing chronic conditions and promoting overall wellness
- Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment
- Document all patient interactions, interventions, and progress in electronic health records
Skills:
- Strong knowledge of human anatomy, physiology, and medical terminology
- Experience working with individuals with developmental disabilities or special needs
- Familiarity with electronic health record systems (e.g., Athenahealth)
- Knowledge of occupational health practices and protocols
- Understanding of ophthalmology procedures and assessments
- Ability to work independently and make sound clinical judgments
- Excellent communication skills, both verbal and written
- Experience working with children is preferred
- Proficiency in discharge planning and coordinating post-hospital care
- Basic life support (BLS) certification
