We are seeking a new Home Health RN to join our team. Your role will be to The Home Health RN will be doing the initial patient intake, as well as follow up visits, for various care from wound care, medication
management, physical and occupational management, and more..
As a member of the Administrator and Clinical department, you will work with Other Home Health Aides, Doctor, and the Key Administrator, Social Worker and report to our Home Health RN.
What You Will Do
- Build and maintain relationships with clients and key personnel within our client companies.
- Escalate and resolve areas of concern as raised by clients.
- Conduct regular visits
- Daily communication, verbal and written with Administrator
- Manage a team of health aides ( LVN, CNA, CHHA, PT, OT, ST, LSW) for excellent and immediate patient care
