**Job Overview:**
We are seeking a dedicated Licensed Practical Nurse to join our healthcare team. The ideal candidate will provide direct patient care under the supervision of registered nurses or physicians, ensuring the well-being of hospice patients in various settings including facilities and homes.
**Duties:**
- Administer current nursing practice following a comprehensive assessment and te plan of care.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Collaborate and participate with healthcare team members (interdisciplinary team) to provide comprehensive patient care
- Uses and prepares equipment and supplies effectively, adhering to aseptic technique.
- Able to quickly adapt to schedule changes as needed.
- Performs other duties as assigne by the Registered Nurse or Leadership.
**Experience:**
- Previous experience in a hospice, home health, home care or healthcare setting preferred
- Knowledge of medical terminology and procedures
- Ability to perform clinical duties outlined in the patient's plan of care.
- Familiarity with case management principles and end of life care a plus.
- Experience in facility, hospital, primary care, oncology or OR settings is a plus. Three years is preferred.
- Currently licensed as an LPN in the State of South Carolina.
- Demonstrates good verbal and written communication, and organization skills.
- Must be a licensed driver with an automobile that i insured in accordance with state and/or organization requirements and is in good working order.
- Posesses and maintains current CPR certification.
- Understanding of anatomy and physiology
