Skilled Nursing Facility seeking a Social Services Director

Brought about by our excellent internal career growth track, this is perfect for the social services professional seeking a solid workplace with a culture of teamwork, respect, and appreciation. Our Skilled Nursing Facility offers long/short-term care, secured memory care, rehabilitation, respite services, bariatric care, and Inpatient therapy.

OVERVIEW:

The social services director will work with residents in the facility to identify their psychosocial, mental, and emotional needs, along with providing, developing, and/or aiding in accessing services to meet those needs. In addition, the SSD is responsible for fostering a climate that enables residents to maximize their individuality, independence, and dignity. This shall provide residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.

The ideal candidate will possess the following:

  • 1 year of social services experience in a Skilled Nursing or Long-Term Care setting preferred.
  • Desire to be part of team that works, grows, and excels …. together.
  • Manage and direct the Social Services Department, and identify and provide for each resident’s social, emotional, and psychological needs.
  • Manage department to assure all psychological and psychosocial needs of residents are met
  • Understand the MDS process with ability to interview, input, and complete social service assessments
  • Accurately assess, document, and provide interventions to mee psychosocial needs of all residents; develop care plans based on assessments
  • Provide effective facilitation of resident care conferences: schedule, send letters to families/ responsible parties, and participate in care conferences
  • Facilitate all resident discharges (arrange for home health, DME, assist with transfer to other facilities)
