Responsibilities:
- Patient Assessment and Care Planning:
- Conduct comprehensive health assessments for residents upon admission and on an ongoing basis.
- Collaborate with interdisciplinary teams to develop individualized care plans based on resident needs and goals.
- Medication Administration:
- Administer medications in accordance with physician orders and established protocols.
- Monitor residents for any adverse reactions and document medication administration accurately.
- Health Monitoring:
- Monitor and record vital signs regularly.
- Conduct regular health checks and assessments to identify changes in residents' health status.
- Communication and Collaboration:
- Communicate effectively with residents, their families, and the interdisciplinary healthcare team.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
- Patient and Family Education:
- Educate residents and their families about health conditions, medications, and self-care strategies.
- Provide ongoing health education to promote resident and family understanding and involvement in care.
- Documentation and Compliance:
- Maintain accurate and up-to-date electronic health records for each resident.
- Ensure compliance with all relevant healthcare regulations and facility policies.
