Bedrock Healthcare is Hiring a Licensed Practical Nurse (LPN) Near Hartford, WI

Watertown HealthCare Center is a leading provider of long-term care services committed to delivering compassionate and high-quality healthcare to our residents. We are currently seeking a dedicated and experienced L.P.N to join our team and contribute to the well-being of our long-term care residents.
Responsibilities:
  • Patient Assessment and Care Planning:
    • Conduct comprehensive health assessments for residents upon admission and on an ongoing basis.
    • Collaborate with interdisciplinary teams to develop individualized care plans based on resident needs and goals.
  • Medication Administration:
    • Administer medications in accordance with physician orders and established protocols.
    • Monitor residents for any adverse reactions and document medication administration accurately.
  • Health Monitoring:
      • Monitor and record vital signs regularly.
      • Conduct regular health checks and assessments to identify changes in residents' health status.
  • Communication and Collaboration:
    • Communicate effectively with residents, their families, and the interdisciplinary healthcare team.
    • Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
  • Patient and Family Education:
    • Educate residents and their families about health conditions, medications, and self-care strategies.
    • Provide ongoing health education to promote resident and family understanding and involvement in care.
  • Documentation and Compliance:
    • Maintain accurate and up-to-date electronic health records for each resident.
    • Ensure compliance with all relevant healthcare regulations and facility policies.
