- Provide on-going training to case managers in the recognition and
management of chronic medical conditions. (health promotion) - Coordinate and integrate disease self-management activities (improve

centers, hospitals, other providers, etc., and intervene when appropriate.

management) - Consult with community agencies and families to maintain coordination of
the treatment process.(referral to community and social support services,
individual and family support services) - Ensure that the team is meeting the goals of the Health Home program.

- The ideal candidate must be licensed a RN in the state of employment and be CPR and First Aid certified. Must hold and maintain a valid driver’s license. Experience and understanding of chemical addiction is preferable. In addition to meeting the qualifications, the ideal candidate will embody the following characteristics and possess the knowledge, skills and abilities listed below: A
