Home Health Coordinator
Pay Range: $25-28/hr

Hours of Operation:
Facility is open Monday-Friday 5am-1pm
Hours of work potentially flexible/negotiable.

BHG is the largest network of Joint Commission accredited outpatient opioid treatment and recovery centers operating almost 120 locations in 24 states, and counting. Responsibilities
  • Provide on-going training to case managers in the recognition and
    management of chronic medical conditions. (health promotion)
  • Coordinate and integrate disease self-management activities (improve
    integration within general health promotion practices). (care coordination)
  • Provide effective discharge planning implementation/ continuity of care.
    (care coordination, comprehensive transitional care)
  • Ensure quality communication with CMHOs, federally qualified health
    centers, hospitals, other providers, etc., and intervene when appropriate.

  • Develop and maintain working relationships with primary and specialty
    care providers, including inpatient facilities. (comprehensive care
    management)
  • Consult with community agencies and families to maintain coordination of
    the treatment process.(referral to community and social support services,
    individual and family support services)
  • Ensure that the team is meeting the goals of the Health Home program.

  • Educate community health referral sources; perform clinical screens; and
    participate, organize and execute care coordination and prevention
    activities. (health promotion)
  • Design and develop prevention and wellness initiatives. (health
    promotion)
Minimum Qualifications
  • The ideal candidate must be licensed a RN in the state of employment and be CPR and First Aid certified. Must hold and maintain a valid driver’s license. Experience and understanding of chemical addiction is preferable. In addition to meeting the qualifications, the ideal candidate will embody the following characteristics and possess the knowledge, skills and abilities listed below: A
