Job Title: Case Manager
The Case Manager is responsible for working with referral sources and clients to ensure access to treatment when appropriate- completing intake, assessment, and admission paperwork. Facilitating educational classes as assigned. Assisting the Lead Case Manager in maintaining maximum client enrollment in the program and assisting with the coordination of client admissions. The Residential Case Manager shall utilize interactive videoconferencing as applicable.
Work week includes 32 hours per week on site with 8 hours non-reporting.
Duties:
- Accept and process all client referrals for residential services
- Conduct intakes on all referrals
- Coordinate client admissions with the Program Supervisor, SUD Counselor and Medical Manager
- Assist with assessments and group counseling as necessary
-Accompanies clients to/from outside appointments as necessary
-Maintain case management documentation and accountability
-Facilitate client education classes as assigned
- Participate in the on-call rotation
-Keep and record program statistics/demographics
- Participate in clinical supervision in accordance with licensure requirements with the SUD Clinical Supervisor, or other appropriately credentialed professional- as assigned.
- Maintain 60% caseload productivity
- Complete diagnostic assessments, treatment plans, and progress notes within 24 hours of the activity
Experience:
-Bachelor’s Degree in Social Work or other closely related field preferred
-Associate Degree in Social work with relevant experience may be substituted for a Bachelor’s degree.
-Must be eligible for a minimum of a CDCA and/or QMHS licensure
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational skills and attention to detail
