Education And Certification Requirements
- High school Degree or equivalent
- Understanding of Medical Terminology
- Minimum of one year of experience, preferably in a community health setting.
- Must have valid driver’s license and satisfactory driving record.
- Strong computer skills with proficiencies Electronic Health Records (EHR) NextGen preferred, Excel, Outlook, Word, and PowerPoint.
- Capable of working with diverse social, racial and ethnic groups and collaborates with professionals from many different specialties.
- Ability to prioritize and plan work activities, use time efficiently and develop efficient workflows.
- Ability to identify and resolve problems in a timely manner and gather as well as present information when needed.
- Ability to adapt to changes in work environment, including adapts to the changing needs of the clinical setting.
- Use of computer and telephone.
- Some lifting of supplies and equipment up to 50 lbs.
- 25% walking or standing, 75% sitting at a desk.
- Category II re: exposure to blood, bodily fluids, and communicable diseases.
- Follow all guidelines and polices set forth in the LCH&D Clinical Policy #500.19 entitled “Medical Patient Tracking Protocol.”
- All aspects of navigation of the referral from scheduling to closing the loop with a transition of care document.
- Obtaining prior authorizations when needed
- Assisting providers with stat referrals when needed
- Provides feedback and offers solutions to the Lead Patient Care Navigator to improve overall referral navigation and racking performance
- Ensuring accurate and timely documentation of all communication with patients in their medical record.
- Ensure that the appropriate provider receives reports or results on each referral tracked.
- Effective communication regarding order status with provider per Medical Patient Tracking Protocol.
- Maintain timeline of open orders, cancelling expired order per provider’s direction according to policy.
- Execute accelerated Referral Navigation efforts when needed such as stat referrals and immediate transition of care (consult note) document retrievals for LCH&D Pharmacy purposes.
- Assist the patient to obtain external referral appointments by daily monitoring of external referral reports
- Maintains updated roster of chosen specialty partners with criteria for referrals
- Collaborates patient care with other healthcare professionals within and outside of LCH&D.
