Here’s how you’ll move healthcare forward in this role
- Take part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety.
- Assess and evaluate the needs of patients with multiple disease processes and comorbidities and develop an appropriate plan of care
- Provides education and strategies for patients and families
- Partners with other VNA clinicians and external providers to execute best patient outcomes
- Documents all changes in patient’s condition and updates care plans and outcomes
- Active Registered Nurse license. BSN a plus.
- 1 year of supervised clinical experience as a registered nurse, preferably in home health or medsurg
- Excellent verbal and written communication skills. Solid interpersonal and time management skills
- Strong computer skills
- Strong problem solving skills
- Car and valid driver’s license
