The PositionAs we continue to grow, we are looking for “SNF-ists,” full-time psychiatric nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting and bring quality mental health services to those communities.
What You Will DoAs the psychiatric nurse practitioner, you will act as a Skilled Nursing Facility’s leading provider of psychiatric services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents.
You will work with the entire facility’s patient population - short and long-term residents - overseeing their care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.

As a psychiatric nurse practitioner, you will provide types of care, including but not limited to:
- Evaluation of new-admission visits
- Coordination of discharge as needed
- Monthly stability visits for long term residents
- Evaluations for patients exhibiting changes in condition
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
- Prescribe all necessary medications and treatments
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with attending physicians and/or specialists regarding patient care
- Proficiency with documentation and CPT coding
- Competitive Remuneration Package- Potential Income Range starting at part time with $65,000- and full time up to $195,000 per year.

- Starting average pay for part time $65,000 up to $130,000/year for full time
- Advanced Practice Licensure (APN) with an applicable state license
- PHMNP certification
- Must have an active license and DEA #
- Previous experience in nursing or other medical fields
- Minimum one-year experience in a Skilled Nursing setting (preferred)
- Ability to build rapport with patients and their loved ones
- Ability to thrive in a fast-paced environment
- Excellent written and verbal communication skills
- Strong leadership experience with emphasis on teamwork in a healthcare setting
- Master’s degree required
