rovision of nursing services aimed towards enrolling hospice patients in accordance with accepted standards of practice and guidelines. You may perform nursing services in a variety of care settings and/or roles, including but not limited to case management, admissions, or on-call. *Benefits:* * Medical/Dental/Vision * _*Company paid*_ life insurance. * _*Company paid*_ long term disability. * _*Company paid*_ long term care coverage. * Pet insurance * 403(b) with generous company match *(Vested immediately) * * CTO accrual starting on day 1! * Paid parental leave. * Additional supplemental insurances! *How you will make an impact:* * You will complete a comprehensive and initial and ongoing assessment of: * Patient and family to determine hospice needs * The impact of the terminal diagnosis on the patient’s physical, functional, psychosocial grief, cultural and spiritual implications, and verbal non-verbal communication patterns * You will assist in developing, implanting, evaluating, and updating the plan of care and make recommendations plan revisions as needed * You will work with the interdisciplinary team to prioritize and provide patient and family-oriented hospice services. * You will provide end-of-life care including comfort care and symptom management according to NorthStar Care Community indicators *What you need:* * Graduation from an accredited school of nursing with possession of a current license to practice as a registered professional nurse in the State of Michigan. * Minimum of one (1) to two (2) years of varied nursing experience required.
