Healthcare Support is Hiring a Nurse Practitioner (NP) Near Newark, NJ
Daily Responsibilities for Nurse Practitioner (NP):
- Provide preventive care to members in their homes using Assistant and EMR systems.
- Conduct thorough reviews and assessments of members' medical history, physical health, environment, and psycho-social status.
- Document progress notes, prepare member workups, and compile encounter summaries using Assistant and EMR.
- Assist at-risk members in transitioning from hospitals or other facilities back to their homes.
- Develop personalized care plans in collaboration with the members' primary care physicians.
- Actively address gaps in care by ordering labs, prescribing medications in collaboration with the primary care provider or collaborating physician, and coordinating follow-up appointments.
- Perform point-of-care labs and diagnostic tests, including A1c and spirometry.
- Support Care Visit program goals by ensuring accurate documentation of member health conditions, achieving high performance on federal quality measures, and facilitating referrals to network providers and internal clinical teams to enhance care and outcomes.
- Coordinate and communicate with members' interdisciplinary care teams.
- Participate in regular ongoing training sessions and meetings, both onsite and remotely.
- Must be based in the state of New Jersey
- Certified as an Advanced Nurse Practitioner in specialty areas
- Hold current unencumbered NJ Licensure as a Registered Nurse
- Active DEA and CDS registration
- Minimum of two (2) years experience as an Advanced Registered Nurse Practitioner
- Master’s degree in advanced clinical practice nursing or graduate of post Master’s certificate program for Nurse Practitioner
- Current provider card in Basic Life Support (BLS)
- Willingness to travel to the member’s home and therefore must have reliable transportation
