COMMUNITY HOSPICE is Hiring a Hospice Nurse Near Newaygo, MI

Hospice RN

Community Home Health Care Services is seeking compassionate and caring Registered Nurses to join our team of home health and hospice providers. We provide high-quality one-on-one patient care, educate, and guide our hospice patient as well as their family/significant other. We strongly believe that continuity of care is important. We are currently offering part-time and PRN positions. We provide in home Hospice care to patients in Newaygo, Lake, Osceola, Mecosta, and southern Wexford counties.

Requirements:
- Graduate of nursing school and currently holding a RN license.
- 1 year of previous nursing experience, preferred
- Previous home health care / hospice care experience, preferred
- Ability to communicate effectively and work as a part of an interdisciplinary team
- Strong ability to use critical thinking and deduction when performing job tasks and evaluating patient needs
- Ability to consistently connect with patients one-on-one
- Responsible, compassionate, and caring demeanor

Job Responsibilities:
- Providing services in accordance with the patient’s Plan of Care
- Performing initial evaluations, building Plan of Care, and regularly reevaluating patient’s nursing needs
- Initiating appropriate preventative and rehabilitative nursing procedures
- Preparing clinical and progress notes for each patient visit via EMR
- Preparing summaries of care conferences on patients in a timely manner
- Informing Physician and Director of Clinical services of changes to patient’s condition and/or needs as appropriate
- Counseling the patient and family/significant others in meeting nursing and related needs
- Supervising and teaching other nursing personnel and CNAs/HHAs
- Completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition

Job Types: Part-time, and PRN

Pay: $40.00 - $90.00 per visit (compensation is based on experience)
