Join our team as: Health Navigator –RHWCWhat You Will DoUnder the immediate supervision of the Clinic Coordinator the Health Navigator provides direct service to community members focusing on COVID 19 Mitigation activities, while being based at and operating from the Wellness Center, delivering services onsite and in the community.
This position is a fixed term contract thru March 31, 2025. Job Functions
- Assist in developing and implementing the COVID – 19 Mitigation program in collaboration with the Rising Harte Wellness Center.
- Oversee the creation and implementation of the COVID-19 Mitigation curriculum.
- Provides health related education workshops in the community and one-to-one education in the clinic regarding the services offered in the Clinic.
- Facilitates client access to care in a timely and cost-effective manner.
- Collaborates and communicates with clients, caregivers/family support persons, and other providers to promote wellness, resiliency, and empowerment ensuring clients’ access to appropriate health services to maximize benefits.
- Serves as an advocate for clients; as an active client of the interdisciplinary team; and as a liaison with other programs and external health and social service providers in the community.
- Perform other related duties as assigned.
Salary RangeIn accordance with California law, the expected salary range for this California position is between $22.55 and $36.73 per hour. The actual compensation will be determined based on experience and other factors permitted by law. This is a COVID-19 Mitigation Grant position, which will end on March 31st, 2025. What You Bring to The Table (requirements):
- High School Diploma or GED.
- Experience providing health education or working in a community setting.
- High school graduate or GED.
- Ability to complete the training program and ongoing educational requirements as
- Ability to work flexible hours.
- Ability to work collaboratively with a diverse team utilizing effective communication skills

- Multilingual capabilities.
- B.A. Degree.
- Prior experience as a Community Health Worker, Peer Support Specialist, Medical
