Key Responsibilities:

Pre-Admission Steps:

• Evaluate inquiries to guide individuals through the care access process.
• Conduct Admission assessments with potential patients or their representatives.
• Explain and ensure understanding of admitting processes.
• Understand clinical criteria for admission to Rogers’ programs.
• Describe Rogers' levels of care to potential patients or their representatives.
• Collect demographic and insurance information.
• Utilize and explain Rogers’ contracted payor listing.

Documentation:

• Ensure all inquiries are entered accurately into the system.
• Capture all necessary information for each inquiry.
• Enter referral source information for each inquiry.
• Enter all clinical information accurately into admission assessment tool.

Performance Standards:

• Meet department standards for call quality and productivity.

Resource Referral:

• Connect potential patients and/or their representatives with outside resources when needed.

Collaboration:

• Collaborate with internal departments and external facilities/referents for access to care.

Schedule:

• Monday to Friday, Day Shift: 10 am - 6 pm.
• 40 hours a week, 1.0 FTE.
• Participation in Rotation for on-call, weekend, and holidays.

Requirements:

• High School Diploma required.
• Bachelor’s degree preferred (in a human services related field).
• Minimum of 2 years’ experience in a health care setting strongly preferred.
• Customer service experience required.

• Positions available for Oconomowoc, WI, and Schaumburg, IL offices.
