Responsibilities
- Communicates with care managers, caregivers, office staff, clients, family members and other medical professionals about client's needs and concerns
- LPN State License - MD
- Current and valid CPR/ BLS Certification
- TB vaccination, Fully COVID Vaccinated and MUST BE BOOSTED and Hep B vaccination
- Comfort with technology; prior experience with electronic medical records preferred
- Ability to drive and travel to cases as assigned
Job Types: Part-time
Pay: $36-40/ hour
