Our Skilled levels of care offers: Rehab, LTC, and Memory Care Rehab. Our nurse leadership team is composed of 4 nurse managers working under the Director of Clinical Services. The entire leadership team works hand-in-hand with a skilled scheduler, Human Resources, and the Executive Director. In order to provide the best care, our team meets weekly to continually improve patient care, associate training, associate retention, associate engagement, associate training, and more. Qualifications:

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • 1-3 years nursing service administration experience
  • 1 year nurse management experience
  • Rehab experience preferred

Job Summary

Oversees the day-to-day delivery of health services provided by the community.

Essential Functions

1. Assesses the health, functional and psycho-social status of new residents and participates in the development of personal service plans.

2. Coordinates healthcare delivery for each community to determine appropriate care through management of clinical services and assessment processes. Participates in personal service plan reviews with the management team, communities and residents’ families.

3. Regulates the medication and health-related supply distribution process, including appropriate documentation, ordering, storage, security, destruction and pharmacy consultations.

4. Ensures the proper and timely documentation of health care according to Brookdale policies and procedures and in accordance with federal and state requirements.

5. Delegates nursing tasks to direct care staff in compliance with the Nurse Practice Act in areas of basic health care and proper documentation.

6. Delivers health-related in-service and medication management training.

7. Assists Executive Director in communicating with family members regarding the changing health care needs of residents.

8. Consults with health care specialists to identify necessary ancillary services and support.

9. Collaborates with Executive Management to implement company policies and procedures to guide the delivery of health care services to communities.

10. Participates in company quality improvement activities and quality assurance surveys.
