Responsibilities:The Long-Term Care (LTC) Behavioral Health (BH) Care Manager contributes to the LTC coordination process by performing face-to-face and telephonic assessments to identify, evaluate, coordinate, and manage members needs including physical health, behavioral health, social determinants of health and LTC services and supports. The BH Care Manager will work closely with the Member’s family and providers to determine the appropriate level of care to meet treatment needs. The BH Care Manager is responsible to ensure that treatment delivered is appropriately utilized and meets the Member’s needs in the least restrictive, least intrusive manner possible.
The incumbent will be accountable for the following:
  • Completes in-person comprehensive assessment of environmental, Psycho-social and support needs. Identifies problems/barriers for care coordination and appropriate care management interventions.
  • Works with member to develop a person-centered plan of care to assist members in reducing/resolving problems and or barriers so that members may achieve their optimal level of health.
  • Identifies and implements the appropriate level of behavioral health intervention based upon the member’s needs and clinical progress.
  • Coordinates care and services with the member, family members as appropriate, PCP, Specialist, and Facility/Vendor Providers.
  • Identify, coordinate, and assist participants in gaining access to needed LTC services and other Covered Services, as well as non-covered medical, social, housing, educational, and other services and supports.
  • Conducts in-person reassessment annually and/or more frequently as needed in accordance with ACFL LTC contract requirements for level of care, significant change, and care plan updates.
  • Works with the participant to complete activities necessary to maintain LTC eligibility and Level of Care.
  • Complete Condition Specific Assessments and refer members to appropriate clinical care and/or behavioral health network provider.
  • Work with utilization management and transition of care departments to coordinate care.
  • Support LTC case managers with behavioral health education and training.
  • Works collaboratively and in alignment with other clinical and social services across the continuum of care striving to enhance the quality of Member care and services.
Education/Experience:
  • Bachelor's degree in social science field including behavioral health, Psychology and/or sociology required.
  • Graduate degree preferred.
  • State licensure preferred; LCSW, LPC, LMHC.
  • Minimum 2 years Behavioral Health experience.
  • Minimum 2 years Case Management experience preferred.
  • Medicaid Managed Care experience preferred.
  • PC Skills and Experience utilizing Microsoft Office, Outlook, Excel, and PowerPoint.
  • Ability to provide constructive feedback with a focus on improved quality.
  • Ability to communicate in a positive/professional manner both orally and written.
  • Ability to work independently, complete multiple tasks in the allotted time.
  • Strong problem-solving skills and decision-making skills.
  • Ability to work effectively in a team environment.
Other:
  • This requisition is in anticipation for future hiring state-wide. Ideally, applicants should reside in AHCA Regions G & I (Palm Beach & surrounding counties & Miami-Dade & Monroe counties). Also, AHCA Regions D & E (Tampa & Orlando Metros).;
  • Must have the ability travel up to 75% of the time.
  • Must have a valid driver’s license, automobile insurance and reliable transportation.
