Role Description

This is a full-time on-site role for an Assistant Director of Nursing at Dignity Home Health and Hospice located in Salt Lake City, UT. The Assistant Director of Nursing will be responsible for overseeing the clinical staff, ensuring compliance, providing medical assistance and support, and collaborating with our team to ensure the highest standards of care is met.

Willing to train the right person - experience in home health and/or hospice is preferred.

Location: Salt Lake City, UT

Responsibilities:

• Collaborate with our team to help with ideas, quality, programs and other ways to continue providing the best care to our patients.
• Help educate field clinicians who are providing care to our patients.
• Review clinical assessments completed upon admission, hospitalization, evaluation, and discharge (OASIS & HIS data).
• Review and analyze utilization, patient plans-of-care, goals and interventions.
• Review clinical notes, ensuring the best practices.
• Stay up to date on clinical programs, policies, processes and changes
• Interact well with physicians, discharge planners, patient, family, team members and other healthcare professionals.
• Manage nursing staff, including recruitment, training, and performance evaluations.
• Ensure compliance with all regulations and laws related to resident rights and nursing practices.
• Oversee the delivery of quality patient care in accordance with established policies and procedures.
• Monitor and maintain accurate medical records and documentation.
