Compass Health is experiencing significant growth across its clinics and is seeking to enhance its team to provide continued support to clients in our service areas. A sign-on bonus of $3,000 is being offered to successful candidates.
### Role and Responsibilities
ResponsibilitiesAs part of our integrated care team, you will provide direct clinical and medical services to individuals enrolled in our Health Care Home program, utilizing a whole person approach for up to 250 clients. Key responsibilities include:
- **Health Screening & Education**: Conduct health screenings for assigned clients and provide ongoing monitoring and education tailored to their medical needs.
- **Treatment Planning**: Collaborate in developing treatment plans and healthcare goals for individuals managing co-occurring chronic diseases.
- **Consultation & Collaboration**: Offer education and consultation to community and office-based staff regarding identifiable health conditions, and coordinate with medical providers on significant health information.
- **Data Monitoring**: Use Health Information Technology (HIT) tools to monitor treatment and medication alerts, as well as hospital admissions and discharges.
- **Reporting & Analysis**: Review and analyze monthly care manager reports that focus on targeted populations and indicators; communicate findings with providers, clinicians, and clients.
- **Screening Coordination**: Organize Metabolic Syndrome Screenings and ensure proper follow-up for those with positive results.
- **Medication Reconciliation**: Perform medication reconciliations for individuals discharged from the hospital who are enrolled in the Health Care Home program.
- **Holistic Service Integration**: Interface with other human and social service agencies to deliver comprehensive services to clients, ensuring culturally competent practices that meet the diverse needs of consumers and their families.
- **Travel Requirements**: Travel to neighboring clinics will be necessary for nursing coverage and training as required.
- High School diploma or GED is required.
- An Associate's or Bachelor's degree in nursing is required.
- Current licensure as an LPN or RN is mandatory.
