PeaceHealth is seeking an experienced full-time Hospice Nurse Practitioner to join a vibrant multi-disciplinary team in Southwest Washington. The PeaceHealth Hospice Care team is dedicated to easing the fears, anxieties, and discomforts that occur during the advanced stages of illness. The team provides a full continuum of care for patients and their families. Medical, spiritual, and emotional assistance helps patients and their families to ease the symptoms of life-threatening illnesses. This opportunity will require traveling primarily throughout Clark County. Ranked the #1 Hospice Care Program in the area with a vibrant outpatient team with four ARNPs and one MD with a growing census of 400 patients. This position will be represented by the WSNA Union.
Highlights about the practice opportunity:
  • Schedule: Full-time, Monday through Friday with Telemedicine incorporated, and mileage reimbursement.
  • Seeing 4-8 patients daily with evening and weekend call requirements, 1:6.
  • On-site complex case management team, social workers, chaplain, including the Ray Hickey and Richard Nau Hospice Houses, and a close partnership with the Palliative Care team. Support from multiple trained staff members and the opportunity to grow the service-line within the region.
  • Provides consultation on patient care and management to other clinicians whose patients are in hospice care, as well as to members of the hospice interdisciplinary team. Assists in evaluating the provision and scope of medical services.
  • Maintains compliance with regulatory changes, documentation requirements, and all procedures related to end-of-life care.
