Southview Acres Health Care Center is recruiting for an MDS Coordinator to become a part of our team. As an MDS Coordinator (LPN), you will be responsible for overseeing and assuring the completion of the RAI/MDS process. This position is responsible for Medicare and MDS process.
Additional responsibilities of the MDS Coordinator include:
- Conducting Patient Assessments
- Developing Patient Care Plans
- Making Patient and Facility Recommendations
- Overseeing Medicare and Medicaid Programs
- Providing Facility Support
- Associate's degree (Bachelor's preferred)
- LPN License in good standing on the MN Health Board
- MDS certification
- Medical assessment
- Patient care
- Attention to detail and critical thinking
- Organization skills
- Communication skills
