Community Team Lead
Full Time/ $45-$48k annual salary

Responsibilities may include:
- Managing a team of TBS Specialist by providing professional support, training and mentorship.
- Managing a caseload of clients at 37.5% productivity.
- Connecting clients with community resources.
- Ensuring that the treatment/behavior plan is being followed.
- Ensuring all required documentation is completed and sent to the appropriate supervisor.
- Communicating and following up as needed regarding the client's status.
- Working collaboratively with those providing therapy, counseling and/or psychiatric services.
Requirements for this position include:
- Bachelor's degree in Social Work or a related field preferred
- Experience in community-based case management.
- Managerial experience is preferred.
- Valid driver's license, car insurance and reliable transportation.
- Willingness to travel throughout assigned service area. This position will require regular travel to Lima, Rockford, Paulding and Hicksville.
