Overview: We are seeking dedicated and skilled Registered Nurses (RNs) for home health positions across Long Island. These roles involve providing high-quality care to patients in their homes, ensuring positive outcomes, and supporting the overall health and well-being of our clients.
Key Responsibilities
- Case Management: Open cases with comprehensive assessments and develop individualized plans of care tailored to each patient's needs.
- Patient Care: Provide appropriate treatments aimed at achieving positive patient outcomes, ensuring the highest standards of home health care.
- Supervision: Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), ensuring they deliver care according to established standards and protocols.
- Patient and Family Education: Educate patients and their families to enhance understanding, improve outcomes, and ensure ongoing effective care.
- Team Collaboration: Work closely with other healthcare team members and administrative staff to ensure coordinated and efficient care delivery.
- Documentation: Complete all required documentation accurately and in a timely manner, maintaining detailed patient records and care plans.
- Licensure: Must be a Registered Professional Nurse with a current license to practice in New York State.
- Experience: At least 1 year of nursing experience is required. Home care nursing experience is a plus.
- OASIS Experience: Familiarity with the Outcome and Assessment Information Set (OASIS) is highly desirable.
- Med-Surge Experience: Experience in medical-surgical nursing is a plus.
- Specialized Skills: Experience with wound care and PleurX catheter management is desired.
- Mobility: Ability to travel between patients' homes within the specified areas.
