GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES:
The Case Manager will provide help to pregnant women and children in getting the health care and social services needed. The person occupying this position will teach women how to take care of themselves and their new families. The Case Manager will also provide emotional support when needed and will link women to needed services. The Case Manager will work with the mother during pregnancy and continue to work with the new mother and her family to promote positive parenting skills and well childcare. The Case Manager is also responsible for maintaining a caseload of active clients by conducting outreach activities and utilizing the case finding list to obtain names of eligible clients.

· work with the mother to reduce stress and help explain identified medical conditions for further evaluation, diagnosis,
and treatment
· responsible for maintaining a caseload of approximately 100 active clients by conducting outreach activities and utilizing
the referral system from High-Risk Infant Follow-Up Program from the State of Illinois.
Requirements
The case manager must meet one of the following qualifications:
Possess a master's degree or baccalaureate degree in a behavioral science, social science, or health-related area; or a baccalaureate degree in any other area and one year of experience in providing direct professional child, family, or community services; or an associate degree and two years' experience in providing direct professional child, family, or community services. Case managers meeting only this qualification must be supervised by an appropriate case manager meeting the requirements of subsection A or B of this Section until they have a total of two years of supervised case management experience.
