Responsibilities:

As a Home Health RN with Sunbelt Staffing, your responsibilities will include:

• Conducting comprehensive assessments of patients' health status and needs in their home environments.
• Developing individualized care plans tailored to each patient's unique condition and goals.
• Administering medications, treatments, and wound care as prescribed by physicians.
• Educating patients and their families on disease management, medication administration, and self-care techniques to promote optimal health outcomes.
• Utilizing Homecare Homebase software for accurate and efficient documentation of patient assessments, interventions, and outcomes.
• Ensuring compliance with OASIS documentation requirements, maintaining a 24-hour turnaround time.
• Achieving weekly productivity targets of 25-30 units, with flexibility for potential on-call and weekend shifts as needed.

Requirements:

To qualify for this position, candidates must possess:

• An active RN license in the state of Florida, in good standing.
• A minimum of 1-2 years of recent experience in home health nursing.
• Proficiency in using Homecare Homebase or similar electronic health record (EHR) systems.
• Demonstrated ability to meet deadlines and maintain a high level of accuracy in documentation, particularly with OASIS assessments.
• Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
• A valid driver's license and reliable transportation, as home visits are required.
• Successful completion of background checks, drug screening, and medical document reviews in accordance with Joint Commission standards.
