We are seeking a dedicated and experienced Staff Pharmacist to join our team for overnight shifts at our long-term care facility in Grand Prairie, Texas. As an Overnight Staff Pharmacist, you will play a crucial role in ensuring the safe and effective medication management for our residents during the nighttime hours. Key Responsibilities:

• Review and verify medication orders for accuracy and appropriateness.
• Prepare and dispense medications according to established protocols and regulations.
• Conduct medication reconciliation and review for new admissions.
• Provide clinical consultations and support to nursing staff and other healthcare professionals.
• Monitor patient medication therapies and provide interventions as needed.
• Ensure compliance with all state and federal regulations, as well as facility policies and procedures.
• Maintain accurate and complete documentation of all medication-related activities.
• Respond to emergency situations and provide critical medication management support.

Shift:

• Monday - Friday: 12:00am - 6:00am
• Rotating weekends

Qualifications:

• Pharmacy experience.
• LTC experience a plus.
• TX Pharmacist license.
