Schedule: Monday-Friday 8:00am-5:00pm- Community Outreach in partnership and co-located with Grants Pass Police Department
Participation in coverage for a rotating 24-hour afterhours line is required and compensated.
Overview
The Community Outreach Case Manager works alongside Grants Pass Police to engage proactively with community members who may have behavioral health needs. The case manager works to connect individuals with the resources they need including mental health and substance abuse treatment services available through Options. The Community Outreach Case Manager is knowledgeable of and works with social agencies and service providers in the community for the benefit of the people they serve. This position works directly in the community (downtown area, parks, community partner locations etc.) both with local law enforcement as well as independently to connect and engage those in need of behavioral health and other services.
Responsibilities
This position provides services which include: advocacy, crisis prevention and de-escalation, skills training, community navigation including connections to services and resources, recovery support, case management, transportation to appointments or connection to transportation resources, collaborative meetings with treatment teams and other external support. Drives agency vehicle and transports clients as required. Submits timely documentation of services provided using Electronic Health Record.
Qualifications
Bachelor’s (BA/BS) degree in a human services field OR three years of experience providing services in a similar setting with individuals who have emotional and/or mental and/or developmental disabilities, OR a combination of education and experience that provides the desired skills, knowledge, and abilities required to perform the job.
Must be a QMHA on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.

Candidates must be able to use e-mail, compose documents, save and locate documents electronically, and learn to use our Electronic Health Record (EHR) system. Proficiency in Word and other MS Office applications preferred.
Must pass both a state-required background check and a modified law enforcement background check, and DMV checks; possess a valid OR driver's license and a satisfactory driving record to transport clients. Candidate must be able to work independently and flexibly, under general supervision.
