Physical Therapist - Home Health - (PT - HH)

Available Shifts: 8 D
Pay Rate: $1815.53 - $2091.80
PT with Home Health experience needed for 13 weeks in Hyannis, MA - ASAP start
** EPIC experience preferred **
Prior home visit experience required.
Be able to Eval and PT care starts as well
Bachelor of Science Degree or Masters Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association.
Current PT license in Ma required.
Current BLS certification required.
Start of care as well as case management of a home based caseload of a variety of patients - up to 6 units each day
Required Qualifications
Home Health Experience
MA License in Hand
** EPIC experience preferred **
