Job Description:
COVID-19vaccination is recommended but not required. Flu vaccination will be requiredOctober through April. Flu exemption applications must be received and approvedby Phoenix Childrens prior to starting. Anyone not vaccinated for eitherFlu or COVID-19 will be required to sign a Declination & Masking Protocolagreements and must mask during the respiratory virus season.
Primarily M-F, flexible hours,with a weekend rotation in the event they cant get all their hours during theweek (wont happen often).
Job Summary:
This position provides physicaltherapy services to pediatric population, neonate to adolescence, includingevaluation and treatment, using modalities such as exercise, heat, water,electricity, massage, light and ultrasound.
Experience Requirements:

• 2 years pediatric experience (school experience will not be considered) required.
• Critical Care experience preferred.

Education Requirements:

• BA or higher degree from a schoolaccredited by the American Physical Therapy Association

Certification/License/Registry Requirements

• Arizona State Board of PhysicalTherapy license prior to start date.
• AZ Department of Public Safety (DPS) Fingerprint Clearance Card (FCC) prior to start date.
• BLS certification from theAmerican Heart Association prior to start date.
