This position is included in a bargaining unit of Housing Works' New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union ("RWDSU"). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a "CBA") between Housing Works and the RWDSU
Compensation: $24-$25 an hour commensurate with experience
Overview:
The Care Manager works with clients and the client's network of providers to help minimize barriers to care and help clients achieve improved health outcomes. The Care Manager will provide case management services for justice-impacted individuals enrolled in our Transitional Housing Program experiencing housing insecurity or chronic homelessness with complex medical and behavioral health needs. Using interventions such as care coordination, motivational interviewing and health promotion, the care managers helps clients, over the course of time, develop the skills needed to manage their health and psychosocial needs with greater independence. The Care Manager will have the opportunity to work as part of outcome driven integrated team and participate in agency wide quality improvement activities aimed at improving the delivery of care to individuals living with chronic illness, behavioral health issues, and homelessness. The Care Manager will have case management experience. This position will be co-located at our Fresh Meadows, Queens location with will travel within Queens to two other transitional hotels and may travel to Manhattan as needed for client coordination and team meetings, trainings, etc.
Responsibilities:

• The Case Manager is responsible for assessing a members needs and coordinating with the member, members family, and network providers to develop and execute an Individualized Plan of Care
• Maintain documentation and update assessments and plan of care at specific time intervals or as needed in Housing Works and Health Home electronic medical records system
• Work and communicate effectively with interdisciplinary team of providers
• Schedule and attend integrated case conferences every 120 days
• Work with the member to identify barriers to goal attainment and help resolve those barriers, i.e., keeping scheduled appointments with appropriate service providers, medication regimen for adherence while promoting optimal health
• Conduct field work to members home and other provider settings, as needed to fulfill plan of care
• Coordinate transitional care from an inpatient service to other settings as per the agreed upon plan
• Work collaboratively with team members to provide outreach and engagement to members
• Provide assistance through advocacy and education to member and family for entitlements and other community services
• Participate in staff meetings, trainings, and supervision
• Consistently exhibit Housing Works core values of teamwork, advocacy, and good customer service
• Performs other duties as requested

Minimum Requirements
Qualifications/Requirements:

• Bachelor's Degree plus 2 years related experience preferred. Masters Degree in related field may be substituted for 1 year experience.
• Adept with word processing, Excel, and Microsoft Office products; Previous experience with Electronic Health Records a plus
• Ability to communicate well both written and oral
• Good clients organizational and multi-tasking skills
• Positive attitude and adaptable with both staff and clients
• Highly organized with the ability to work independently and prioritize multiple responsibilities in a fast-paced environment
• Comfortable working with a diverse group of staff and clients
• Understanding of chronic medical conditions, homelessness, substance use and mental illness
• Knowledge of City, State, and Federal entitlement and benefit systems.
