Lutheran Social Services of Illinois is a not-for-profit social service agency with more than 70 program sites throughout the state of Illinois. The full-time Care Coordinator Supervisor will be responsible for the daily oversight of the Care coordination team which includes high fidelity wrap around care coordinators and intensive care coordinators. Additionally, this position supports Care Coordinators in their day-to-day processes and job responsibilities by providing oversight of the delivery of care coordination services, training to assigned staff members and assists in care coordination services to children and families with behavioral health needs. The Supervisor will guide staff in the utilization of evidence-based practices (High Fidelity Wraparound) that are a team-based, strength-based care approach to service planning and coordinating care.
