Responsibilities:

• Records & reports observations.
• Conducts telephone follow-ups.
• Performs other related functions as requested by the supervising nurse within the scope of practice and related experience.
• Participates in performance improvement activities.
• Performs other job related duties as may be required or directed by the nursing supervisor.

Requirements:

• Must be fluent in conversational Spanish.
• Participate in performance improvement activities.
• Performs other job-related duties as may be required or directed by the nursing supervisor.
• Must be COVID vaccinated.
• Home care experience a PLUS.
