7a to 5:30p 4 / 10 hour shifts Assesses and plans care that effectively manages a patient along the continuum in an efficient/cost effective manner while assisting in directing certain aspects of care from pre-admission (when appropriate) to post discharge. Performs admission and continued stay review of patients, using Severity of Illness/Intensity of Service Criteria to determine appropriateness of admission, location/level of service (inpatient, observation, outpatient), and continued stays in the hospital. Assesses and identifies patient's needs and assists patients/families/significant others with discharge planning. Ensures that the outcomes of patient care are within the appropriate length of stay, that appropriate/effective resources are used, and that established standards are met through collaboration with the health care team and the patient/family/significant other. Education REQUIRED: Registered Nurse (RN) with current active nursing license in the state of Alabama required. Excellent oral and written communication skills required. Experience REQUIRED: At least 3-5 years of nursing experience required. Licenses/Certificates REQUIRED: Current nursing license in the state of Alabama and in good standing with the Alabama Board of Nursing INDAL
