SOCIAL WORKER
The Hamptons Center for Rehabilitation and Nursing is actively seeking an experienced Social Worker to work full-time at our Skilled Nursing Facility in Southampton, NY.
DUTIES:
- Maintaining and/or improving residents ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in the planning of the residents admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
REQUIREMENTS:
- Bachelors Degree in Human Services; Psychology or Social Work is a Plus
- Long Term Care/ Subacute Experience Required
- Discharge Planning Experience is a Plus

Board Certified physicians and licensed skilled nursing staff offer specialized care to all residents. Our experienced rehabilitation specialists work closely with each individual to help meet their goals for recovery.
The position offers excellent benefits, including medical & dental coverage at no cost for the employee, generous PTO, and a 401(k) plan with employer contribution.
Hourly Rate: $32 - $35
