Job Summary:
We are seeking a dedicated and experienced Occupational Therapist to join our team at CRS Home Therapy, P.C. as a Full Time Home Health Occupational Therapist. As a Home Health OT, you will be responsible for providing in-home occupational therapy services to patients in the Mchenry, Illinois area. This is an individual contributor role, where you will have the opportunity to make a positive impact on the lives of patients by helping them improve their functional independence and quality of life.

Responsibilities:
- Conduct initial evaluations of patients' condition and develop personalized treatment plans.
- Visit patients' homes to provide occupational therapy services and monitor their progress.
- Administer therapeutic exercises and activities to improve strength, range of motion, and functional abilities.
- Educate patients and their families/caregivers on safe and effective techniques to carry out daily activities.
- Document patient progress and treatment plans in accordance with state and federal regulations.
- Collaborate with other members of the healthcare team to ensure coordinated and comprehensive care for patients.
Requirements:
- Active and unrestricted Occupational Therapy license in the state of Illinois.
- Minimum of 2 years of experience as a licensed Occupational Therapist, preferably in home health.
- Strong communication and interpersonal skills to effectively work with patients, families/caregivers, and other healthcare professionals.
- Ability to work independently and manage a caseload while meeting deadlines.
- Knowledge of and ability to follow state and federal regulations related to occupational therapy services.
