The role you’ll contribute:

Coordinate initial patient referral information from any setting: physician office and/or healthcare facility. Key aspects include patient and caregiver involvement; care planning for start of care, treatment, and services; clinical communication education of home health service availability.

The value you’ll bring to the team:

  • Comply with home health policies, procedures, practices, and care standards as witnessed by leadership and staff.
  • Coordinates intake staff ensuring they complete their duties and coaches those needing additional mentoring to achieve their potential.
  • Respond to requests for home health care from referring sources within the community.
  • Initiate, maintain, and document ongoing communications with the patient, patient’s family/caregivers, and physician to assess and implement referral and /or hospital discharge needs.
  • Utilize current and accurate knowledge of resources available within the community for referrals accepted and not accepted.
  • Provide needed information to Central Billing Office for centralized verification of insurance coverage, initial preauthorization as required, and inform staff, patients, caregivers and POAs as appropriate.
  • Assure concurrent information of payor source, planned services, and diagnosis to facilitate anticipated reimbursements from all payor sources for post-acute care services.
  • Communicate orders and other pertinent information to agency staff and supervisors who will be implementing the plan of treatment.
  • Assemble face to face documentation and enter encounter information into EMR.
  • Assemble admissions packets with pertinent medical information for clinical staff.
  • Scan and attach clinical documentation to clinical record as appropriate.
  • Demonstrate knowledge and skills to perform intake duties, methods and procedures required by the job.
  • Participate in orientation, in-service and staff meetings as scheduled.
Qualifications


The expertise and experiences you’ll need to succeed:

KNOWLEDGE AND SKILLS REQUIRED:

  • Strong evaluation and social services skills required.
  • History of positive interpersonal and communication skills required.
  • Able to communicate effectively in English, both verbally and in writing.

EDUCATION AND EXPERIENCE REQUIRED:

  • Bachelor’s Degree
  • Minimum of 2 years of health care experience
  • Experience with computers required.

EDUCATION AND EXPERIENCE PREFERRED:

  • 2 Years of Home Care Experience
  • Experience with Epic software preferred.
