Qualifications:

• Undergraduate Degree or equivalent experience
• A minimum of 2 years experience in the long-term care or insurance field with an understanding of nursing home reimbursement, Medicare advantage plans and ISNP is preferred
• Insurance experience is a plus
• Knowledge of state and federal requirements for long term care facilities, as appropriate
• Keen attention to detail coupled with strong organizational and communication skills
• Experience interfacing via phone, TEAMS, or face to face within the healthcare or health services industry
• Knowledge of managed healthcare principles
• Must possess intermediate level of proficiency in Microsoft Office, PowerPoint, Excel, Word and Adobe Acrobat

Responsibilities:

• Maintains and assures compliance with Value Based Insurance Plans to provide regular onboarding, resident eligibility, and enrollment. Ensure established goals are successfully met.
• Monitors and trains on each community’s ISNP plan or valued based product to ensure accuracy
• Meets regularly with Director of Complex Care Management to ensure established goals and targets with ongoing projects are met
• Ensures that insurance consultants and other support resources are appropriately utilized
• Establish, maintain, and coordinate a strong working relationship with Community staff, Saber and LTC staff, and external business partners
• Represents Director of Complex Care Management at meetings when delegated
• Travel for meetings/reviews/etc. as delegated by Director of Complex Care Management up to 35% of the work time
• Hosts, leads, and gives presentations at meetings as delegated by Director of Complex Care Management
• Other duties as deemed necessary and appropriate, or as directed by the Director of Complex Care Management
