Overview:
Commonwise Home Care is seeking a dedicated individual to join our team as a Nurse Educator. In this multifaceted role, you will be instrumental in building connections within the Lynchburg community, focusing on educating and supporting older adults and their families about aging, home care options, and resources available for aging in place or transitioning to senior living communities.
The Care Management aspect of this role also includes providing senior housing & relocation assistance, supportive consulting, family relationship management, healthcare crisis management, coordination of healthcare, home safety evaluations, and life care management to clients. They also serve as account managers for clients receiving home care services and nurse-aides providing their direct care.
This role truly is the best of both worlds for those passionate about the nursing field and networking in order to understand the needs of the aging adult community.
Guaranteed at least 10 hours a week.
