Summary :

Provides comprehensive pharmaceutical care to CCH patients, including drug information, therapeutic drug monitoring and pharmacokinetic dosing, renal dose adjustment, therapeutic interchange, IV to PO conversion, and patient education / counseling.Serves as a subject matter expert for the healthcare team regarding drug therapy.Advocates safe medication use.Aligns prescribing, dispensing, administration, and monitoring functions with established policies and procedures for safe practice.

Required Education :

Bachelor’s degree is required.

Master's degree is preferred.

Doctoral degree is preferred.

X Other:Experience in hospital or similar Pharmacy setting preferred.Strong computer skills essential.Ability to communicate clearly in written and oral forms required.

Required License:

NCBOP Pharmacist Licensure

Required Certification:

BCLS within 30 days of hire.
