JOB DETAILS

POSITION TITLE: Fatherhood Coordinator

DEPARTMENTl Clinical Services

FLSA STATUS: Non-Exempt

REPOTRS TO: : Assistant Director of Operations

GENERAL DESCRIPTION OF DUTIES

The purpose of this position is to support the Agency’s needs by servicing as the Fatherhood Coordinator for its Fatherhood Initiative Program. The focus of this position is to promote responsible fatherhood through the provision of workshops, groups, and other direct services, using an evidence-based curriculum and actively participating in the Fatherhood Strategic Framework.

SPECIFIC DUTIES AND RESPONSIBILITIES

ESSENTIAL JOB FUNCTIONS

The list of essential functions, as outlined herein, is intended to be representative of the tasks performed within this classification.   • Regular attendance
  • Attend and complete all mandatory training.
  • Conduct community outreach and participant recruitment to reach members of the target population of low- income fathers raising children in the CRT catchment area.
  • Plan, coordinate, and facilities the advisory group provider meetings, including the recruitment of community providers to attend the meetings, and making the necessary arrangements to find guest speakers to present at the meetings.
  • Facilitate designated curriculum workshops and support groups.
  • Conduct a fatherhood assessment and develop a service plan using the CRT Steps to Success case management system, which identifies needs and goals related to employment, education, and other social determinants of health.
  • Create and maintain up to date paper and electronic client records to include but not limited to case notes, group notes, sign in sheets, pretest, post-tests, evaluations, and certificates of completion.
  • Prepare and submit timely reports.
  • Participate in community meetings and outreach activities.
ADDITIONAL JOB FUNCTIONS
  • Perform all other duties as assigned
  • Must be available to work late evenings, weekends, and unconventional hours.

MINIMUM TRAINING AND EXPERIENCE

Education: High School Education Required, must be a self-identified as a Peer relating to the content as described.

Minimum Years of Experience: One (1) Year of experience in working with families, fathers, or participating as a peer within the family’s social services system required

Active Certification in good Standing: Must have valid driver’s license.

Demonstrate skills in : Excellent written and verbal communication skills. Basic MS Office and computer skills for reporting and data entry.

Knowledge: Knowledge of Hartford areas where clients who are not receiving services. Proven experience working with families, fathers within the family’s social services system required.

Bilingual: Yes, preferred Fluent Speak: English Written: English

Active Driver License in good standing required upon hire and throughout employment.

Physical Ability: Tasks involve sedentary to light work, involving some reaching, handling, fingering and/or feeling of objects and materials.

Sensory Requirements: Some tasks require visual perception and discrimination. Some tasks require oral communications ability.

Environmental Factors: Tasks are regularly performed without exposure to adverse environmental conditions, such as dirt, dust, pollen, odors, wetness, humidity, rain, fumes, temperature and noise extremes, machinery, vibrations, electric currents, traffic hazards, animals/wildlife, toxic/poisonous agents, violence, disease, or pathogenic substances.
