North Country HealthCare AZ is Hiring a Program Specialist/Health Educator (HealthySteps) Near Winslow, AZ

This is a part-time position, 20 hours/week in Winslow, AZ

General Description of Position:

The Program Specialist/Health Educator: Develop and sustain relationships with organizations, health centers and community centers, maintaining a consistent presence in the community and collaborating to identify potentially useful resources for families with children 0-5 years. Manage the data entry duties and monitor documentation of service delivery, program processes and other documentation related duties. Community referrals and follow-up activities. Facilitate play groups and teach classes to families and staff. Perform developmental and sensory assessments. Coordinate HealthySteps service delivery The specialist will work closely with the pediatric provider team in both Navajo and Apache County, serving as the link between the medical provider and families with children 0-5 years old. The specialist will support the primary medical provider by bringing specialized knowledge to non-medical problems in the area of child development and behavior and all other duties required.

Essential Duties and Responsibilities:

  • Monitor and oversee operating results of program service assignment and take steps to correct problems.
  • Prepare and recommend program services operation to management. Compile, review and analyze operations reports. Make recommendations for management evaluation.
  • Identify and interpret needs for community and/or program development. Provide leadership in coordinating activities of agencies, groups and individuals to meet identified needs. Study and assess strengths and weaknesses of existing resources. Maintain contact with representatives of community groups, of other agencies, or public officials, and of community-based organizations to exchange and update information on resources and services available.
  • Write proposals or assist in the writing of proposals to obtain government or private funding for projects to meet community needs.
  • Supervise activities of the assigned program and ensure the best program delivery as possible.
  • Participate as a member of the regional management team in regional and agency planning, monitoring and evaluation.
  • Keep up-to-date and knowledgeable of programmatic issues, ensure compliance with all applicable laws, regulations, funding requirement, assurances, policies and procedures.

Education and Experience:

  • High School Diploma or GED
  • 3-Years Related Experience and/or training

Requirements:

  • Cultural Competence with Native American and Hispanic cultures
  • Travel required

Preferences:

  • Bachelor's Degree
  • English/Spanish bilingual
  • Experience in Early Child Development

Compensation:

Starts at $19.52/hr DOE
